What are the different types of Liver transplants carried out at BGS Gleneagles Global Hospitals?
Orthotopic Transplant
An orthotopic transplant is procedure where the whole diseased liver is removed and replaced with the donated liver from a cadaveric donor. During the procedure, the surgeon makes an incision in abdomen. The diseased liver is removed and replaced with the donor healthy liver. The donor liver is then connected to the recipient’s blood vessels and the bile ducts. Finally, the incision is closed with suture and then covered with sterile dressing.
Living Donor Transplant
A living donor transplant procedure involves removal of a part (right or left lobe of liver) of healthy liver from a living person and then transplanted to the patient. The donor must be the patient’s close family member. During the procedure, the surgeon makes an incision in abdomen. The diseased liver is removed and replaced with the donor healthy liver. The donor liver is then connected to the recipient’s blood vessels and the bile ducts. Finally, the incision is closed with suture and then covered with sterile dressing. After transplantation, the donor’s remaining lobe and transplanted lobe quickly regenerates themselves.
Split Liver Transplant
A split liver transplant procedure involves transplanting right and left lobe of the liver from a recently deceased person to an adult and a child respectively. That means, the donated liver of a deceased person is split into two for transplantation. The larger right lobe is transplanted to adult and the smaller left lobe is transplanted to a child. During the procedure, the surgeon makes an incision in abdomen. The diseased liver is removed and replaced with the donor healthy liver. The donor liver is then connected to the recipient’s blood vessels and the bile ducts. Finally, the incision is closed with suture and then covered with sterile dressing. After transplantation, the transplanted lobe quickly regenerates itself.

ERCP
Endoscopic retrograde cholangiopancreatography (ERCP) is diagnostic and treatment procedure for bile and pancreatic duct problems. ERCP is suggested if the bile or the pancreatic ducts become blocked or narrowed due to gall stones, infection, chronic pancreatitis, and tumors in the pancreas and bile ducts. ERCP uses a thin, flexible, optic tube called endoscope. Anaesthetic spray, gargle, or lozenge is used to numb the throat and to relax the gag reflex. Then, the scope is inserted through the throat and oesophagus to the point where the bile ducts from pancreas and gall bladder drain into the duodenum. A catheter is inserted through the scope and a contrast material is injected into the bile and pancreatic ducts. Then, several x-rays were taken simultaneously to detect the abnormalities. The doctor may insert small tools which allow obtaining a tissue sample, opening a blocked or narrowed bile duct, placing a stent, or removing gall stones.
Laparoscopic Cholecystectomy
Cholecystectomy is the removal of gall bladder. It is indicated in the patients with painful gall stones. Laparoscopic gall bladder removal is a minimally invasive procedure where small incisions are made and fine surgical instruments are used to remove the gall bladder. General anaesthesia is given before the surgery. A small incision is made in the abdomen and a cannula is inserted. A laparoscope that consists of tiny camera is inserted through the cannula to view the internal organs. The surgeon inserts other cannulas to gently separate the gall bladder from its attachments and removes it through the opening. Then the incision is sutured and covered with sterile dressing.
Liver Resection
Liver resection is the most common type of liver surgery performed. Liver resection is indicated in the patients with malignant liver tumor. General anaesthesia is given before the surgery. During the procedure, the part of the liver that is affected with cancer is removed along with the surrounding liver tissue. If the cancer is present in the right side of the liver, then gall bladder is also removed to avoid complications. The surgery may take two to five hours. The liver resection increases the survival rate of the patients with liver cancer or colorectal cancer.
